Emergency Contacts
Must have up-to-date details for your emergency contacts
Must have at least 2 names other than the principal caregiver
Your tamaiti/tamariki will not be allowed to leave KK unless accompanied by one of the above or someone authorised by the mātua/kaitiaki

Custodial Statement
Any parenting orders must be made known to management
A copy of any court orders must be signed and a copy held of file by Kura Kārearea.
A person's name must be given to Kura Kārearea is NOT allowed to collect your tamaiti/tamariki

Fee's
All invoices must be paid one week in advance
Any bookings made up to 7 days prior, must be paid asap
Matua must advise Kura Kārearea if your child is absent by 9am.
Charges will still apply unless your tamaiti is mauiui or attending tangihanga.
I agree to pay a late fee if my tamaiti is picked up outside the booked hours. There is a grace period of 15mins, anytime over this will roll over to the next scheduled hour.
I agree to give 2 weeks' notice in writing when canceling my tamaiti's booking.
I agree to pay a late fee if my tamaiti/tamariki is left at Kura Kārearea outside of operating hours (6pm for Afterschool Care and 5:30pm for School Holidays).  This will be charged at $1 per minute.

Health
In the event of a medical emergency, I understand my child will be taken to the hospital in an ambulance if necessary. The mātua or approved contact person will be notified immediately. All medical costs incurred are at the child's principal expense.
I give permission for Anica cream to be applied to bumps and bruises.
I give permission for staff to apply sunblock to my tamaiti/tamariki skin
I give permission for my tamaiti/tamariki to applybasic first aid treatment by staff
Medications and Allergies
All medications administered to your tamaiti/tamariki by Kura Kārearea staff must be handed to the supervisor on arrival and must be signed in at the medical register by a mātua or kaitiaki
Any allergies must be made known (food or otherwise). 

Unacceptable Behaviour
Please advise management if there are any behavioral issues we need to know about. We have ZERO tolerance for physical violence, foul language, and intentional damage to the kura or its equipment. If your child's behavior is considered a risk to others, you will be notified and asked to remove your tamaiti. If there are any further incidents, your tamaiti may be excluded from future attendance at Kura Kārearea.
When you confirm making a booking with Kura Kārearea, you agree to accept the cost of any damage wilfully caused by your tamaiti and you agree to pick up your child immediately if his/her behavior is deemed unacceptable.

OCSAR Subsidy
We are OSCAR accredited and as such, you can apply for an OSCAR subsidy with the Ministry of Social Development (MSD)
It remains your responsibility to ensure your account is paid weekly and pay any pafrt of your fees that are not covered by the subsidy. It is also your responsibility to keep MSD informed of any changes in your circumstances or care arrangements. Always ensure that your application/renewal is lodged with MSD prior to your child attending Kura Kārearea or prior to the expiry of the current subsidy. In confirming your tamaiti/tamariki booking, you agree to pay all your fees, even if your WINZ application is declined.

Activities
Staff will do their best to keep your child safe during activities inside the school grounds. However, Kura Kārearea staff and supervisors will not be held liable in the event of injury or damage or loss sustained to your tamiti/tamariki personal effects that have been brought into the programme.
By confirming your tamaiti/tamariki booking you give permission for him/her/them to participate in activities outside of the kura which may include transport in a Kura Kārearea approved vehicle (car bus etc)
By confirming your tamaiti/tamariki booking you give permission for him/her/them to be transported in a staff members car in the case of an emergency or if he/she/them need to be dropped off at home if other arrangments have fallen through.
By confirming your tamaiti/tamariki booking you give permission for him/her/them to be photographed during Kura Kārearea programmes. These photographs/videos may be used on Kura Kārearea's website or social media for publicity purposes.

Agreement Confirmation
By confirming your tamaiti/tamariki booking you confirm that you have read the terms and conditions and that in this agreement you declare that the information you have provided is true and correct.

NOTE: information provided in your booking may be required for statistical purposes. As required by the Children Act 2014. The information may be shared with appropriate organisations if there are any concerns about the safety of the tamaiti/tamariki. Kura Kārearea staff and supervisors follow the principles of the Privacy Act 2020.
